Referral & Intake Completion Policy

COMPASSIONATE CARE

Patients are asked to complete all required forms and payment within 14 days of receiving their intake package.

Referrals that remain incomplete for 90 days from the date the intake package is sent will be closed.

Patients who have not been seen in the clinic for more than one year may be asked to obtain an updated referral prior to booking further appointments.

Our Appointment Arrival Policy

Patients are asked to arrive 15 minutes prior to their scheduled Comprehensive Menopause Intake appointment.

If you are running late, please contact the clinic as soon as possible.

Patients arriving late may have their appointment shortened, rescheduled, or considered a missed appointment if insufficient time remains to safely complete the visit.

Cancellation Policy

We require a minimum of 48 hours notice for appointment cancellations.

Appointments cancelled with less than 24 hours notice may be subject to a missed appointment fee.

No-Show & Missed Appointment Fees

$50 - Comprehensive Menopause Intake (Nursing Appointment)

$160 - Initial Physician Consultation

$65 - Physician Follow-Up Appointment

Prescription Refill Policy

Prescription renewals are completed during scheduled appointments to ensure safe and appropriate ongoing care.

The Menopause Clinic does not routinely process pharmacy fax refill requests outside of scheduled appointments.

Patients enrolled in the Continuity Membership may access prescription renewal coordination between appointments when clinically appropriate.

Forms & Administrative Requests

Insurance forms, disability forms, special authorization requests, letters, and other administrative documents are uninsured services and may be subject to fees.

Patients enrolled in the Continuity Membership may have certain routine forms and special authorization requests included as part of their membership benefits.
